 (Schedule may change at any given time based on needs of Individuals served)

DUTIES AND RESPONSIBILITIES: (Must be able to drive/ Valid License with excellent driving record)

  1. DDC is responsible for working with DDP in researching and seeking out new candidates for our program.  To assist individuals without waivers by directing to appropriate agencies for the waiver.

 

  1. Responsible for assisting DDP in the intake of all new consumers.  Responsible for assisting DDP in making sure all paper work pertaining to new consumer is received and completed.  Responsible for placing all information in individuals' books.

 

  1. DDP is Responsible for planning, developing, implementing services provided in a specified Service Program with the assistance of Creative Solutions, RN and LPN.  DDC is responsible for keying in this plan in Residex.

 

  1. Responsible for setting up New Consumers entire profile in Residex.  Service Plan, doctor, family information, etc.

 

  1. Responsible for assisting DDP with billing issues with consumers; contacts the appropriate parties in order to correct any and all billing issues such as; MOA's PA's, budgets, etc.

 

  1. Performs periodic audits of consumer records in office and individuals' homes, committee reports, and in-service manuals to assure conformance with state and federal regulations.

 

  1. service plan ("ISP"), the code of ethics, and the organization's polices and procedures, by visiting consumers twice a month at their residents  and once  a month at Dayhab/Work.

 

  1. Assists DDP assuring quality care is delivered as prescribed in consumers IEP's.  Responsible for visiting schools once and month to make sure IEP's are being implemented.

 

  1. Periodically observes care given by personnel and documents and reports findings to Human Resource Manager and DDP.

 

  1. Assists DDP in assuring consumer assessment and care is within appropriate standards and that the ISP reflects such care.

 

 

  1. Reviews consumer case records for accuracy, current notes, medical records, ISP, DMA 06, DMA 80, supervisory conferences, and supervisory visits. Places all appropriate documents in clients' books office and home and uploads appropriate documents in Residex based on DDC daily assignments and completion of ISPs and etc.  Checks for missing medication, recaps and notes daily.  Notifies Human Resource and DDP of the missing medication notes and recaps.

 

  1. Responsible for assisting DDP in making monthly reports/calls to Individuals Parents notifying them of  Dr. visits/appointments that have taken place and addressing or asking if there are any concerns they may have.  This call should be done at the end of each month.

 

  1. Assist Human Resource and DDP in arranging staff education/training.

 

  1. Encouraging and assisting personnel to continue self improvement through formal education, educational meetings and active participation in professional and related organizations.

 

  1. Assists with planning, and coordinating continuing education programs.

 

  1. Assists DDP in assuring quality care is delivered as prescribed by the attending physician, code of nursing ethics, and the agency's policies and procedures.

 

  1. Must be on-call at least 2 times out of the month for after hour's assistance.

 

  1. Responsible for making sure Individuals get to and from their scheduled medical appointments.  Works   extremely   closely with Office Manager & HHMC with reporting new appointments and making sure appropriate paper work is received and given to Office Manager based on visits past, present and new.

 

  1. Works closely with HHA in making sure that all supplies needed for individuals are acquired.  Assists HHA in acquiring these supplies when necessary food personal items etc.

 

  1. Responsible for making sure all repairs are taken care of at individuals homes.

Supervisory Responsibilities                                                

 Assists with the monitoring of employees in the Direct- Care Staff. Is responsible for assisting in the overall direction, of care of the consumers the  employees deal directly care for, is involved in the coordination, and evaluation of the services provided.  Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include addressing complaints and resolving problems pertaining to consumers or consumer's families.

Education and/or Experience                                             

 Associates Degree from an accredited College or University; or one to two years related experience and/or training in treating/working with individuals with Intellectual Disabilities.  2 years or equivalent combination of education and experience.
